Enrollment Kit

Use the Enrollment Kit to enroll eligible retirees.  The Enrollment Kit consists of three key items:

1.      Description of Plan Benefits and Enrollment Form;

2.      Investment Fund Information brochure; and

3.       Current Investment Fund Overview (updated quarterly).

These materials collectively provide newly enrolling employees with important plan details, including the available investment options.  Section three of your online employer handbook contains more information.

NOTE:  When distributing Enrollment Kits to eligible retirees, be sure to include the most recent Investment Fund Overview.  It is important that retirees receive up-to-date investment performance when making their investment decisions.
